Instructions:

Please fill out the form completely.

Unless otherwise indicated in the "Additional Requests" field, your District Executive will deliver the completed flyers to the schools that you list in this form approximately one week prior to your recruiting event.

NOTE: it may take up to 5 days to produce the flyers. Please order your flyers at least 2 weeks prior to the planned join night.

Order Information

The following information will allow us to properly fulfill your order.

* Desired Quantity of Flyers
* Schools to be Flyered
Please list each school that will be flyered. If you are not distributing these flyers in a school please provide a brief description of how the flyers will be distributed.
Additional Requests
Describe additional information that should be included on the flyers or any special requests (i.e. you would like to pickup the flyers at the council office instead of having them delivered directly to you or the schools).

Additional Recruiting Materials

Please list the quantities for each item you would like to be included with your flyer order.

Posters will include the same information as the flyers.
